It's usually just a misunderstanding of how the process goes.
Most people are used to iTunes and such where you have access to something the second you hit "buy", but in our case there can be a delay (they don't get the email until the payment gets processed, which is out of our hands, really).
There is the occational "something went wrong", but it's probably just some web browser plugin or the like blocking something that needs to be able to run.
